To schedule, reschedule, confirm an appointment or have questions about exams at specific locations, please complete the following form. Please note: A written order from an M.D., D.O., D.P.M., D.C., or your dental provider is required for any diagnostic imaging exam. Xray exams are provided on a walk-in basis only at designated centers Monday – Friday, 8am-5pm. Please click here, for a list of designated centers. Please allow 24 hours for us to respond to your inquiry.
Images and reports are provided to patients via email. If you are a patient and had your exams performed at any Imaging Healthcare Specialists locations please click here to submit a request online for your images. Medical providers will be required to submit a signed authorization from the patient prior to release of images and/or reports. For all other medical records inquiries, please complete the following form. Please allow 24 hours for us to respond to your inquiry.
